Take 5 Book

The Kalamazoo Take 5 Book - also known as a SLAM, Start Card, Stop and Think pad, SafeCheck, Take Time, and other names - is a pocket-sized personal planning tool designed to help eliminate workplace accidents.

The easy to use book is designed to be completed by all workers before commencing a potentially hazardous task. The Take 5 book guides the worker through five clearly defined steps to help identify, then control any potential hazards.
